HEALTH SAVINGS ACCOUNT~Checking Account

• Minimum to open $10.00
• Initial $25.00 HSA set up fee-Paid separately-No Annual Fee
• 1st order of 150 checks are FREE- Style Emerald Single or Duplicate
• FREE - Master Card Debit Card

• Interest begins to accrue on date of deposit.
• Must have minimum balance of $10.00 on day dividend paid (month end) to earn interest.  Interest credited to account monthly.  Interest is compounded daily using the daily balance method on a 365-day year.  Rate may be change as often as daily at our sole discretion.  There is no limit on the changes in the interest rate.  Interest rates are earning on the daily account balance.

  $10.00 - $999.99                 0.75%      0.75%APY
  $1,000.00 - $9,999.99          0.75%      0.75% APY
  $10,000.00 - 49,999.99        1.00%      1.01% APY
  $50,000.00 - $99,999.99      1.25%      1.26% APY
  $100,000.00 - greater           1.50%      1.51% APY
